BIOMETRIC CONSENT AND NOTICE

Soteria Credentialing LLC ("Soteria") uses biometric technologies, including facial recognition and palm vein recognition, to verify identity and control access to healthcare facilities.

By enrolling in or using Soteria's services, you acknowledge and consent to the following:

Biometric Information Collected

Soteria may collect biometric identifiers or biometric information including:

• Facial geometry or facial templates derived from photographs or scans
• Palm vein patterns or palm vein templates
• Associated identity and credentialing information necessary to provide access services

Purpose of Collection

Biometric information is collected solely for:

• Identity verification
• Vendor credentialing
• Facility access control
• Fraud prevention and security monitoring
• Compliance with hospital access requirements

Storage and Security

Soteria employs commercially reasonable administrative, technical, and physical safeguards to protect biometric information, including encryption, access controls, and audit logging.

Soteria does not sell, lease, trade, or otherwise profit from biometric information.

Retention and Destruction

Biometric information is retained only for as long as necessary to provide services to customers. Upon subscription cancellation or expiration, Soteria permanently deletes biometric records in accordance with its Biometric Data Retention and Destruction Policy.

Disclosure

Soteria may disclose biometric information only:

• To authorized hospitals or healthcare facilities using the service
• To service providers operating on Soteria's behalf under confidentiality obligations
• As required by law

Consent

By selecting "I Agree," enrolling in Soteria, or providing biometric data, you expressly consent to Soteria's collection, use, storage, and processing of biometric information for the purposes described herein.
